Canceling Appointments:
If you can't keep your scheduled appointment,
please call us as soon as possible.
This courtesy makes it possible to give the time
to another patient. A 24-hour cancellation notice is required.
A $20.00 fee will be charged for no-shows.

The patient-doctor relationship is unique, sacred and confidential.
Your right to privacy and confidentiality is very important to us.
All information provided will be held in strict confidence.
We will not release any information without written permission
from the individual or responsible party.
